The strategy worked: Lovato cut out enablers and found love with then-boyfriend Wilmer Valderrama, 38, whom she dated for nearly six years before they split in June 2016.
"(We) started dating when I turned 18. Right after that, I went to rehab," Lovato said in her 2017 YouTube documentary Demi Lovato: Simply Complicated.
"People told him, 'You should probably leave. She's on a spiral, and you're going to be sucked down with it,' " she said in 2015.
"But he was like, 'I'm not leaving. This is somebody I really care about.' "
Thanks to that support, Lovato made her way back, appearing on The X Factor while in her sober-living house and releasing three chart-topping albums.
Headlining sold-out arena shows helped Lovato find her voice as an activist, speaking out for mental-health causes, women's issues and campaigning for Hillary Clinton.
She also became a co-owner of the CAST Centers, the Los Angeles mental-health-and-wellness treatment facility where she initially sought help.
But the nonstop pace of her career ran headlong into efforts to find inner peace.
"She needs to get out of this whole system that she's been in since she was 13," says a former member of the singer's inner circle.
"Demi never had a day off." Like many former child stars, she is also the breadwinner in her family and ultimately calls the shots.
"Her mother is a very sweet person who has zero say," says the source.
"Even at 16, one day Demi's like, 'I'm paying the bills of this house. This is my house.' Dianna loves her daughter and wants her to get better.
But if she wants to survive, she needs to get out of this business. Right now her health and her life are at stake."
Since the overdose, Lovato's mother, stepfather Eddie De La Garza and sisters Dallas and Madison are doing their best to be of help.
Valderrama has also been by Lovato's side since her hospitalization.
Although the pair aren't back together, "he's been her rock through some of her darkest moments," says a pal close to the former couple.
"He's the love of her life and vice versa." Loved ones want what's best for Lovato, but the next steps are up to her.
"The thing about an addict is that you become so good at hiding it," says the longtime friend.
"In the end she has to want to get help. She can't be forced into treatment. It won't work." For now, the circle around her is guardedly optimistic.
"It's too soon to say what the next steps are in terms of treatment," says an insider. "Everyone just wants to help get her to a healthy place."
